Attending Physicians
Remains the patient’s primary care consultant and works as part of the hospice team.
Medical Director
Certifies the attending physician’s diagnosis and prognosis for the patient, leads the hospice team in developing the plan of care, and coordinates with the attending physician.
Registered Nurse
Evaluates the patient’s condition during regularly scheduled visits. Coordinates the plan of care with the interdisciplinary team, family, and patients and teaches the family about proper patient care, the use of medications, and pain/symptom management.
Licensed Skilled Nurse
Licensed nurses work with the hospice care team to provide pain and symptom management and comfort measures to your loved ones.
Certified Home Health Aide
Supports your loved ones by providing assistance with personal care and with activities of daily living such as ambulation, transfers, toileting, bathing, dressing, and feeding.
Social Worker
Social Worker will evaluate the patient’s and family’s psychosocial needs and concerns, will provide emotional support and active listening, and will assist with advanced directives and final arrangements.
Spiritual Counselor
Our non-denominational chaplain makes initial contact with each patient and their family to assess and give spiritual counsel. Works with the clergy of their choice as liaison. Assists with funeral and memorial preparations.
Trained Volunteers
Provide non-medical friendly visits and respite to patients and their families. They give needed emotional support and encouragement.
