Learn About Fortitude Hospice Care and Services

The hospice program is designed to cope with the special needs of patients who are terminally ill by providing them with personalized, individualized, and compassionate care and services through pain and symptom management and comfort measures, along with the psychosocial and emotional needs of their families and responsible parties. The hospice program provides care, peace of mind, privacy, and dignity that the patients and their families deserve in their own homes or chosen facilities. In addition, the hospice program also provides assistance in preparing for a dignified, comfortable, and peaceful end-of-life event.

Hospice Eligibility Criteria

We determine the eligibility for hospice care when the patient meets the following:

  1. Has limited life expectancy.
  2. Curative measures have ended, and the focus is now on emotional, physical, and spiritual comfort.
  3. The patient or the family or responsible party has opted not to seek aggressive treatment, invasive surgical procedures, and major hospitalizations.
  4. The patient’s attending physician has authorized hospice care.

Who Pays for Hospice Care?

Hospice care is covered by Medicare Part A, Medi-Cal, and by most insurance plans.
